3-501.16(A)(2): (A) Except during preparation, cooking, or cooling, or when time is used as the public health control as specified under §3-501.19, and except as specified under ¶ (B) and in ¶ (C ) of this section, TIME/TEMPERATURE CONTROL FOR SAFETY FOOD shall be maintained: (2) At 5ºC (41ºF) or less. Observed the following issues with cold holding temperatures: 1)TCS foods held in the kitchen refrigerator was 44-48 degrees. This food was moved to the walk-in cooler and the air temperature in the refrigerator dropped to 41 degrees. This is not a commercial quality piece of equipment and should only be used temporarily until the commercial cooler is repaired. Only foods that will be used in the kitchen within a short period of time should be temporarily stored here. 2)TCS foods held in the walk-in cooler were 43-48 degrees. The air temperature at the fans was 45 degrees. After about 30 minutes, the air temperature registered 34-37 degrees. What most likely happened was that the door was not fully closed for several hours and the air temperature rose. Pay special attention to the door and make sure it is always tightly closed. COS |

(B) Label information shall include:
(1) The common name of the FOOD, or absent a common name, an adequately descriptive identity statement;
(2) If made from two or more ingredients, a list of ingredients and sub-ingredients in descending order of predominance by weight, including a declaration of artificial colors, artificial flavors and chemical preservatives, if contained in the FOOD;
(3) An accurate declaration of the net quantity of contents;
(4) The name and place of business of the manufacturer, packer, or distributor; and
(6) Except as exempted in the Federal Food, Drug, and Cosmetic Act § 403(q)(3) - (5), nutrition labeling as specified in 21 CFR 101 - Food Labeling and 9 CFR 317 Subpart B Nutrition Labeling.
(7) For any salmonid FISH containing canthaxanthin or astaxanthin as a COLOR ADDITIVE, the labeling of the bulk FISH container, including a list of ingredients, displayed on the retail container or by other written means, such as a counter card, that discloses the use of canthaxanthin or astaxanthin.
Observed frozen fish, frozen chicken, prepared salsas, and vegetables packaged in the establishment for retail sale with no labeling information. Label all packaged foods as outlined above and maintain by the next routine inspection. |

THE COOLING LOG WAS IN USE EARLIER IN THE YEAR, BUT DOES NOT HAVE CONSISTENT TEMPERATURES RECORDED OVER THE LAST FEW MONTHS. USE THIS LOG TO TRACK COOLING ITEMS EVERY DAY. BECAUSE THERE IS NO CFPM ON STAFF, A THIRD INSPECTION WILL TAKE PLACE BEFORE THE END OF THE YEAR. |
